Pakistan PM Imran Khan, the President Arif Alvi, and the Defence Minister Pervez Khattak have covid19

Summary: Famotidine, ivermectin and supplements - and then steroids at day 7-8 from first symptoms (requires careful counting of first day of symptoms). Required to escape discomfort during disease, and micro-thrombi in organs, and to avoid long haulers syndrome entirely. Please discuss with your physician beforehand.

PM, President, Defence Minister, recently-removed Finance Minister

According to a video message by Pakistan PM Imran Khan, he caught it after a year of precautions, but then he let his guard down during recent Senate election campaigning, and so caught it.

Coincidentally he got his first shot of the Sinopharm vaccine and then tested positive. The PM's wife had also tested positive.

President Arif Alvi also reports Mar 29 that he has covid19. His wife reports her PCR test is negative, and she is in quarantine. She does not report if she is getting any post-exposure prophylaxis treatment.

Meanwhile Defence Minister Pervez Khattak also announced he has covid19.

Many of them got it after getting the first dose of vaccination - this is because the 3rd wave is starting and the immunity from vaccination has not fully kicked in yet (takes many weeks after second jab).

EDIT: it seems the just-removed Finance Minister has also tested positive for covid19: https://tribune.com.pk/story/2292240/former-finance-minister-abdul-hafeez-sheikh-tests-positive-for-covid-19 Former finance minister Abdul Hafeez Sheikh tests positive for Covid-19 March 30, 2021

Sinopharm vaccine

Sinopharm is an inactivated virus based vaccine, which would contain fragments of the virus by design, and be delivered with an adjuvant in the mixture, in order to boost the immune response. However I am not sure if that could be the source of false positives - since presumably the virus debris (which an inactivated vaccine has) would not typically be visibly to a nose swab PCR test. Sinopharm's efficacy may be at the lower end, but for these cases and arrival of the 3rd wave, the vaccine would just not have had time to work - since takes a few weeks after 2nd jab to get full immunity.

From anecdotal reports the Sinopharm vaccine generally leads to no reaction for most, but some runny nose and congestion in others - which can be reversed with Famotidine 20mg - 1+1 for 3 days.

Famotidine is an H2 blocker antihistamine, which is very effective during covid19 disease as well.

My guess is if vaccinations were accompanied by handing out some tablets esp to high risk or weaker older individuals, it could reduce the adverse effects even further - for all vaccines.

The premier had gone into self-isolation at home after testing positive for the virus. His wife and first lady Bushra Bibi was also confirmed to have the virus the same day. On Sunday, Special Assistant to the Prime Minister (SAPM) on Health Dr Faisal Sultan said Imran had made "steady clinical recovery" from Covid-19 and that he had been advised that he could resume work and build up his work routine over the next few days. Following the prime minister's positive test result, the Ministry of National Health Services had clarified that the premier was not "fully vaccinated" when he contracted the virus. "He only got the first dose merely two days ago which is too soon for ANY vaccine to become effective. Antibodies develop 2-3 weeks after the second dose of 2-dose Covid vaccines," it said.

President Arif Alvi and Defence Minister Pervaiz Khattak have tested positive for the coronavirus, just a day after officials announced that Prime Minister Imran Khan is making a a steady recovery from the infection. President Alvi announced his infection in a tweet on Monday. However, his wife Begum Samina Alvi’s test came back negative. It is stated that Alvi has mild symptoms and was in good spirit. “I have tested positive for Covid-19. May Allah have mercy on all Covid affectees. Had 1st dose of vaccine but antibodies start developing after 2nd dose that was due in a week. Please continue to be careful,” the president said in his tweet.

Tweeting world leaders about early treatment

I have tweeted some instructions for them - primarily ivermectin and Famotidine and for day 7-8 steroids - to prevent hyperinflammatory stage, and to avoid micro-thrombi in organs and long hauler syndrome

But I doubt anybody will listen to such suggestions - they will ask their doctor. And anyone searching for ivermectin will only find WHO, FDA and CNN and NYTimes article as fact checkers on Google - these articles are piggybacking on the failed Colombia JAMA paper which has been discredited for it's many flaws.

So you need an attentive and curious person and a half hour to convince them. Otherwise this denial-of-early-treatment story is hard to believe by the public.

I replied using this series of tweets:

Early treatment is essential for risk reduction - zero complications and zero long hauler syndrome In your case count the first day of symptoms very carefully (call it day 1) live viral load is max at day 1 within 5-6 days it has gone down to near zero

https://twitter.com/stereomatch2/status/1376963202236489733?s=19

for nearly all people by day 8 live virus is gone however viral debris is huge this causes continual hyperinflammation visible around day7 visible on oximeter reading declines or pulse rate elevated 90s or 100+ at rest

https://twitter.com/stereomatch2/status/1376963378544009219?s=19

treatment includes: ivermectin - hugely effective Famotidine - reduces symptoms within 24 hours and supplements by day 7 steroids have to be started to quell hyperinflammatory syndrome typically 20-30mg Deltacortril per day for a week then taper over next week

https://twitter.com/stereomatch2/status/1376963497414774788?s=19

this allows body time to clear the debris timely steroids use limits organ damage from micro-thrombi (hyperinflammation leads to endothelial damage leads to release of clotting factors)

Ivermectin and cancer/chemotherapy

To one former Health Minister who seems to be undergoing chemotherapy, I added this tweet on ivermectin compatibility with chemotherapy:

Good wishes to you. Ivermectin is useful as an adjuvant for chemotherapy. AND it is effective against covid19. https://www.reddit.com/r/ivermectin/comments/l3keqr Ivermectin and cancer #BeBraveWHO #Ivermectin #ivermectinworks #covid19

Why have you not heard about early treatments?

Some have made the argument that hostility to early treatment may be an existential issue for some industries. For example many drugs like Remdesivir (which earned $3 billion for Gilead and are still in use, despite having little efficacy according to WHO) - these proprietary drugs would not receive Emergency Use Authorization.

However there are other reasons as well.

Early treatment is relatively unknown because of how hospitals have had to deal with serious cases - mild cases are sent home.

Result: the mild cases sent home this week become the severe cases of next week.

It is such an absurd situation.

That is why you are more likely these days to find a doctor running a small clinic to be more responsive than a big hospital. Or a doctor who does telemedicine and is known to treat patients early.

The problem for the public is at their moment of crisis, they do not know ahead of time if a doctor they are about to see is the early treatment type or not.

Fortunately there are lists of telemedicine doctors doing early treatment now - see the "Doctors doing early treatments" section below.

Early treatments

Check out this post about suggestions for world leaders who get covid19:

https://www.reddit.com/r/ivermectin/comments/mgojzi Pakistan PM Imran Khan, the President Arif Alvi, and the Defence Minister Pervez Khattak have covid19

The situation got to this point by the generally correct presumption that most people will get mild disease.

But what they did not figure then and is known now is that a large portion - possibly 25-30 percent -of covid19 patients get long hauler syndrome.

The NIH in the US has now woken up and allocated $1 billion for studying long haulers - but they are still hostile to recognizing early treatments ever since Hydroxychloroquine split the US politically.

https://www.nature.com/articles/d41586-021-00586-y US health agency will invest $1 billion to investigate ‘long COVID’ 04 MARCH 2021

Here is just one such medicine - which is included in the I-MASK+ and MATH+ protocol mentioned in the above link:

https://www.youtube.com/watch?v=ix8i7dfsCJg Dr Lawrie, Ivermectin video, Short Precis Mar 8, 2021 Dr. John Campbell

Other drugs known to reduce mortality by half - Famotidine (this came out of the early Chinese data - former US president was given this as part of cocktail of drugs). Reduces symptoms within 24 hours.

There are other harder drugs in process - better prescribed by doctors - like Fluvoxamine - though those are promising and just starting to accumulate enough data. The above cheap and safe drugs however on their own do the work well.

Early treatment experience around the world

In some parts of the world, they are being handed out in an early treatment packet of medicines that people can take at home - check the ivermectin subreddit (which covers worldwide trends for early treatment with cheap generic drugs as well).

Here is a hospital in the US offering Fluvoxamine (a promising drug) as early treatment of high risk patients - it has more info on the evidence for Fluvoxamine:

https://www.cityhealthuc.com/fluvoxamine A New Option for COVID-19 Treatment Involving Fluvoxamine

However Fluvoxamine is a harder drug and should be taken under advice of a physician. In comparison ivermectin is available over the counter in many countries.

An example from India using Ivermectin:

https://www.hindustantimes.com/lifestyle/brunch/a-crown-of-care-during-covid-101615047368077-amp.html

The kit contained a thermometer, an oxymeter, five days’ doses of Arithromycin, 10 tablets of paracetamol, three tablets of Ivermectin and 10 capsules of vitamin C, complete with detailed and precise instructions of when and how to consume them.

Some counties in Mexico, Brazil and Peru have mass delivered some of these medicines as mass prophylaxis.

The impact is examined by Juan Chamie - a researcher compiling statistics on the South American experience with covid19:

https://twitter.com/jjchamie?s=09

in this video where he discusses his paper:

https://www.reddit.com/r/ivermectin/comments/lz8f6p/flccc_video_dr_pierre_kory_discusses_with_data/ FLCCC video - Dr Pierre Kory discusses with data analyst Juan Chamie the impact of mass ivermectin use and then discontinuation (due to political reasons) on Mexico City and Peru cases/deaths (March 6, 2021)

Doctors doing early treatments

Early treatment is being done on individual level by physicians, and some hospitals.

The FLCCC website maintains a list of early treatment telemedicine doctors for the public - the links for those are given in this post:

https://www.reddit.com/r/ivermectin/comments/malbqw/telemedicine_doctors_dr_syed_haider_an/ Telemedicine doctors - Dr Syed Haider - an interesting telemedicine doctor and emerging resource for Ivermectin and Fluvoxamine

But the main groups pushing for increased adoption are:

The FLCCC (Front Line COVID-19 Critical Care Alliance)

https://twitter.com/Covid19Critical?s=09

https://covid19criticalcare.com/i-mask-prophylaxis-treatment-protocol/ I-MASK+ for prophylaxis and early treatment

https://covid19criticalcare.com/flccc-protocols-a-guide-to-the-management-of-covid-19/ MATH+ for treatment and hospital care

The MATH+ protocol pdf above gives a good outline of, and reasoning behind, treating with steroids at day 8. Live virus peaks at day 1 of symptoms, and drops to near zero by day 8 for nearly all people. Viral debris is huge though and leads to the post-day-8 hyperinflammatory stage that causes endothelial damage and release of clotting factors.
