The writer is seen is as a critic in the ivermectin community - because of his statements or quotes in news articles against ivermectin - perhaps he was quoted out of context in those articles.

But here in his own article, he gives a fairly balanced view of ivermectin.

It is a good article on ivermectin, including it's safety, and the situations where it is unsafe (when blood/brain barrier is compromised).

Article urges caution before ivermectin is prescribed to general public - but urges trials to be sure. Which is all that he could say.

I would probably add that ivermectin should be avoided if you are going to be doing intense exercise or activity where you could have a minor stroke (haemorrhage) or bleeding in the brain. Or if you drink alcohol or are at risk of weakened blood/brain barrier - since ivermectin is usually prevented from crossing the blood brain barrier but if someone has meningitis (or has a stroke/leakage) then it is advised not to take it.

That is if someone thinks they have covid19 and are taking ivermectin, then behave carefully - dont act like ivermectin is allowing you to act differently than you would during sickness.

And in any case, it is not advisable to engage in strenuous activity - going to the gym for an intense workout or running - if you have a viral infection.

Hey, Stereo, your caution about alcohol: did you get that from Adam Gaertner? AFAIK, alcohol isn't a problem with ivermectin. There are a couple allegations of that posted to the sub, both of which are quoting Adam.

The idea that alcohol weakens the blood brain barrier makes sense - I may have heard it somewhere other than Adam Gaertner.

Other drugs may perhaps be more important - like steroids - which may be one reason Trump was not given ivermectin - if he was too close to day 8 where steroids would have to be given (day 1-8 being viral replication and post-day8 being immune overreaction stage).

Using ethanol with ivermectin can increase the blood levels or add to the side effects of ivermectin. This can cause skin rash, swelling, headache, dizziness, weakness, nausea, vomiting, diarrhea, stomach pain, seizure (convulsions), shortness of breath, and numbness or tingling. Conclusions These results suggest that IVM (30 mg oral, QD) is safe in combination with an intoxicating dose of alcohol, but do not provide evidence that this dose of IVM is effective in reducing alcohol craving or its reinforcing effects. Given the preclinical data suggesting IVM is effective in reducing alcohol consumption in mice, additional studies testing larger samples and alternate dosing regimens are warranted to further characterize the potential efficacy of IVM as an AUD treatment.

Thanks for doing all that research! Very interesting stuff! Too bad ivermectin isn't useful for treating alcoholism.

Googling "does alcohol weaken the blood brain barrier", the top result is:

The results indicate that while ethanol or acetaldehyde alone do not cause a weakening in the blood-brain barrier, the additional stress of food withdrawal after alcohol exposure does reduce barrier function, and this could be significant in human binge drinking.

The next question: does it matter if ivermectin crosses the BBB? Sure, it sounds bad, but I've never seen anything suggesting that it actually happens OR that there was a bad result. Doesn't mean it's not true: what I don't know could fill Wikipedia. It's not clear to me, though, that crossing BBB = huge downside.

Looking for evidence that ivermectin + alcohol is a problem, I found and just posted "Analysis of severe adverse effects following community-based ivermectin treatment in the Democratic Republic of Congo", which found 55 deaths after taking ivermectin (oh no!); alcohol is listed as a factor.
