Early Diagnosis and Early Management Proposal in Dengue Infection: New Normal to Applied Strategy for COVID-19 Infection (Thailand, July 17, 2020)Preprint https://papers.ssrn.com/sol3/papers.cfm?abstract_id=3649412
Abstract:
This proposal consists of 4-dimensional aspects as rehydration with oral rehydration therapy (ORT), blunting the pro-inflammatory cytokines with cytokine inhibitors such as tumor necrosis factor-alpha (TNF-α) and interleukine-6 (IL-6) with pentoxifylline and doxycycline, inhibit DENV replication, viral load reduction and nonstructural protein 1 antigen (NS1Ag) clearance with doxycycline and ivermectin, and restoring the immune function with vitamin D and zinc supplementation. The main advantage of these drugs and supplements is that it is an affordable, already available with an established safety profile, further resulting the possibility of better both clinical and laboratory outcomes as well as reduction in hospitalization and economic cost of treatment. Another advantage is that it may apply this strategy for other virus-induced hypercytokinemia such as Corona Virus Disease (COVID-19) and Ebola. This hypothetical management need to be subjected to randomized, controlled trials to evaluate their ability to early recovery, prevent the disease progression to severe dengue and other recognized complications.
