Exact title is easy. For this, it's "Quadruple Therapy with Ivermectin is effective in treating COVID-19". Usually you can swipe to put it into the clipboard, for pasting when you post the link.
Country is sometimes ambiguous or hard to figure out. If the article is in Hindu Times, it's probably India. To be extra careful, you might notice "Karnataka" in the by-line and look up it's location. India.
For formal studies, look at the authors. This study does not have an obvious country, until you click the authors list. In this case, I might look up "Xi'an Jiaotong University Medical College" and find out its in China. In fact, the study was done in Bangladesh. I was unaware of any Chinese trials so I searched ClinicalTrials.gov and found this trial, which identifies the location as "Chakoria Upazilla Health Complex Cox's Bazar, Bangladesh, 4741". Usually it's not that hard.
If the 4 authors are from 4 countries, and you can't figure out where the trial was actually done -- just go with the 1st author's country.
Date is usually obvious for news stories and less so for studies. If you can't find any date, just use "found <date", so you would append something like (Bangladesh found 2020-09-22) to the title.
Optional comment is whatever you want it to be. Make it useful. I like to put "Results" on posts that report trial results.
Applying the naming convention, the correct title for the Hindu Times article linked in the Exact Title paragraph is:
Quadruple Therapy with Ivermectin is effective in treating COVID-19 (India 2020-09-14)

What country is a paper from? When looking at a formal paper, it's often not clear. In that case, I look at where the authors are from, by clicking the (usually provided) links showing their university affiliation or similar. If there's a consensus, that's the country. If there's no consensus about country but they all are associated with XYZ organization, enter XYZ in lieu of country.
The goal is to identify the paper in a way that makes it easier to find. I have searched the sub for Bangladesh a 100 times. XYZ can help, if no country can be discerned.
