TWiV (This Week in Virology) does a bad job (IMO) of assessing the potential of ivermectin

This is the email that I sent to TWiV:

In my opinion, your podcast on ivermectin badly mischaracterized the potential of the drug.

From this study regarding dose:

The primary safety endpoint was mydriasis, accurately quantitated by pupillometry. Ivermectin was generally well tolerated, with no indication of associated CNS toxicity for doses up to 10 times the highest FDA-approved dose of 200 microg/kg.

That's 2 mg/kg. It is a very well tolerated drug.

This paper got incredibly good results with a single dose of 150 mcg/kg.

Results: The cohort (including 704 ivermectin treated and 704 controls) was derived from 169 hospitals across 3 continents with COVID-19 illness. The patients were matched for age, sex, race or ethnicity, comorbidities and a illness severity score (qSOFA). Of those requiring mechanical ventilation fewer patients died in the ivermectin group (7.3% versus 21.3%) and overall death rates were lower with ivermectin (1.4% versus 8.5%; HR 0.20 CI 95% 0.11-0.37, p<0.0001).

Ivermectin reduced fatalities by 5/6ths.

During your podcast your expert said the long-lasting effect of ivermectin is a mystery. It's really amazing.

This paper from Merck shows that a single dose of ivermectin killed 99.5% of parasite larvae three months after taking the drug and more than 90% killed a year after dosing.

For good reason, researchers are interested in ivermectin.

There are 9 ivermectin trials registered at ClinicalTrials.gov and 2 ivermectin trials at ClinicalTrialsRegister.eu.

Bill Gates is funding a project by a French company that has a device that you insert under your skin. It will emit ivermectin for 3 months. The goal is to prevent catching the virus.

Try again, please.

Interesting thanks for the references. This is relating Episode 599 I suppose. It felt like their takedown was a bit hasty. The slow release is interesting I wonder if topical application with an inhaler would be possible (albeit as she likes to point out it is not approved as that).

I saw an MD's suggestion of using a nebulizer. Not sure how that's different from an inhaler.

The slow release is interesting

Uh, remind me what that is, please?

Ah sorry, i was just referring to the French company design as "slow release". I guess i don't know what the difference between inhaler and nebulizer is but we mean the same thing.. i thinking of that spray like thing one takes their asthma meds with.

Oh, now I get it. Yes, I think the French thing is very interesting -- because it implies (without evidence) that ivermectin prevents catching covid.
